Overview

The Schedule a Test screen allows you to search for a collection site based on an address or zip code and schedule the type of drug test needed.

Navigation

Select the link at the top of the screen or in the drop down menu on the Testing Tab to open the Schedule a Test screen.

Searching for a collection site

  1. Once the Schedule a Test screen opens you must select a Company if your company is not selected for you.
  2. If you select the Cost / Location Code the Address will be automatically populated and the Default collection site will show at the top of the list for that Work Location. NOTE: If your donor is not at that Work Location, it is best not to select this option and just begin entering the address where you are searching for a collection site.
  3. You can enter a full address or just enter a city and state or zip code.
  4. Select the regulatory mode if not already selected.
  5. Select the type of test you want to perform.
  6. Adjust any of the filters to help narrow your search based on certain collection site attributes. Collection sites that can enter chain of custody data electronically are selected by default. This helps to process and report test results quicker than non-electronic collection sites.
  7. Select Search to get the list of collection sites.

After Selecting the Search button the list of collection sites will display sorted by distance from the Address you are searching from.

  1. The Tier and Price columns will show pricing for collection sites that have pricing configured.
  2. The Flags shows D for default collection site when a Cost / Location Code is selected and other flags with an appendix at the bottom of the list.
  3. Selecting the Select link under the Select to Proceed column will take you to the Order a test screen where you fill in the remaining information about the donor.

  5. Selecting the Map Selected Sites brings up a google map with each site selected and the ability to select the site from the mapped pin
  6. Selecting the More Info greater than sign will show the attributes and hours for each collection site.

Scheduling a drug test

Selecting the collection site opens the Schedule Drug Test screen where you begin entering the donor information you are scheduling a test for.


  1. The Cost / Location Code will default to the primary location if not set on the previous search screen but can be changed. The collection site can be changed by selecting the magnifying glass icon next to the collection site name.
  2. The Reason code will default to Pre-Employment and can be changed at this point.
  3. The Expiration date is set to 3 days from today by default but can be changed by changing the number of days and selecting the refresh icon to recalculate the expiration date.
  4. If the drug test is for an existing employee, the link icon can be used to find the employee and add all the donor information automatically.
  5. Donor Id will default to Drivers license for regulated test but can be changed to use SSN or Employee Id. For drivers license, the State and license number are combined to create the donor Id.
  6. If a phone number is entered and the Text checkbox is selected, the authorization form will be texted to the number entered.
